Your Declaration And Important Information

1. I authorize Jachin 333 Pty Ltd (ACN 125 520 078) (trading as Acumen Accounting & Advisory) to act as my registered tax agent for the sole purpose of cancelling my Australian Business Number (ABN) on my behalf.
2. I acknowledge that, in order to process this cancellation request, Jachin 333 Pty Ltd must be listed as my tax agent on the Australian Taxation Office (ATO) Tax Agent Portal. This means they will temporarily become my tax agent by default.
3. I understand that Jachin 333 Pty Ltd does not provide other taxation services, such as Business Activity Statement (BAS) preparation or tax return lodgements. Once my ABN cancellation request has been completed, they will remove my details from their tax agent portal unless I request further services.

1. I authorize Jachin 333 Pty Ltd (ACN 125 520 078) to act as my registered tax agent and submit an Australian Business Number (ABN) application to the Australian Taxation Office (ATO) on my behalf. If required, I also authorize Jachin 333 Pty Ltd to submit a Business Name application to the Australian Securities & Investments Commission (ASIC).
2. I acknowledge that Jachin 333 Pty Ltd will use the information provided in this form to facilitate my ABN application and, if applicable, will continue as my ASIC agent to assist with business name registration and renewal notifications.
3. I confirm that I have obtained consent from all individuals mentioned in this application to share their personal information with Jachin 333 Pty Ltd, the ATO, and ASIC as necessary for processing this request.
4. To the best of my knowledge, all information supplied in this application is true, accurate, and complete.

• I am not disqualified from managing corporations under Section 206B(1) of the Corporations Act 2001.
• Within the past five years, I have not been convicted of or released from prison after serving a sentence for any offences listed under Section 32(1)(c) or (d) of the Business Names Registration Act 2011.
• This application complies with the ASIC Electronic Lodgement Protocol.
• I understand the most suitable business structure for my needs (Partnership, Sole Trader, or Company).
• I possess the required identification details for all key business individuals, including either Tax File Number (TFN), date of birth, and full name OR full name, residential address, and date of birth.
• I am not applying for an ABN solely because an employer or another party has requested it as a condition of employment.
• My business meets (or will meet) key indicators of an enterprise, including:
  • Sourcing my own clients/customers
  • Having the ability to delegate work to others if needed
  • Issuing invoices and determining my own rates
  • Maintaining a dedicated business bank account separate from my personal account
  • Taking responsibility for public liability, professional indemnity, and/or workers' compensation/income protection insurance
• I can provide evidence of my entitlement to an ABN if requested by the ATO.
• I understand that providing false or misleading information in an ABN application may result in penalties of up to $12,600 per offence.
• Once issued an ABN, I must ensure it remains up to date (reporting any changes within 28 days) and cancel it when I am no longer operating or carrying on an enterprise.
